If you are a certified physician or a contracted material supplier, you can apply for an electronic medical report (subject to the Integrated Health Care Regulation) to the Supreme Health Council (SHC) of the National Organisation for Health Care Services (EOPYY) in the following cases:

• the initial medical report is marked as "Exceedance Decision Required (SHC)"
• the filling of the medical report entails "Financial Limit Exceedance"

You will need to:

• enter the details of the initial medical report
• upload the necessary supporting documents to justify the financial limit exceedance
• enter comments supporting your request
