• your Tax Identification Number (AFM)
• your Social Insurance Number (AMKA)
• your email address
• your mobile phone number
• a valid and active bank account number (IBAN), in which you are the sole beneficiary or co-beneficiary

For the final submission of the application, you will need to visit the municipality or community centre and produce the necessary supporting documents.
