Request a disability assessment and certification (KEPA)

You can apply for disability assessment and certification by the Disability Certification Centre (KEPA).

In addition, you can:

  • track the progress of your application
  • be informed of the date of your examination by the competent health committee
  • display the result of disclosure of disability

Applications can be submitted for:

  • Initial assessment for certification
  • assessment for the extension of the certification period
  • assessment in relation to the worsening of an existing medical condition or the addition of new condition
  • Assessment for review of an existing certification

The application can also be submitted by your legal representative, namely:

  • the parents and guardians for their children
  • the guardians for adults
  • the holder of a notarised power of attorney
  • your legally authorised representative

You will need to:

  • enter your personal Taxisnet credentials
  • enter your Tax Identification Number (AFM) and your Social Insurance Number (AMKA)
  • attach the required supporting document that proves your relationship with the person being examined and enter his/her details (applies to legal representatives)

Once your application is filed, you need inform your treating physician about the registration of your disease Documentation Folder.
