You can report a surnamed or anonymous incident of delinquency by or against a minor. The report can be made whether you are a minor or an adult and may concern either:

• to yourself/themselves
• someone you know
• something you've seen or heard

You will need to complete:

• the details describing the incident (when and where it occurred, who is involved, the frequency of the incident and any other additional information deemed necessary)
• your contact details (either your mobile phone number or your email address)
