Complete the electronic health statement for sea travel, within 24 hours before your trip. It is mandatory to fill-in the statement before you embark on ships when travelling from mainland Greece to the islands and vice versa.

You will need:

• your email address
• the details of the ship you are travelling on
• the ports of your departure and destination
• the date and time of travel
• the details of your seat on the ship
