• at the time of submitting your application, a period of at least six (6) months has elapsed since the last assessment of your physical fitness.
• your enlistment has been postponed for health reasons, for a period of one or two years (not six months)

You will need your personal Taxisnet credentials.

You can optionally attach a medical certificate showing improvement or deterioration of your medical condition, or a formal declaration in this respect.
