• clinical examination
• laboratory tests
• radiological or endoscopic examinations
• surgeries
• application of a lawsuit
• inoculation
• medication
• hereditary diseases
• specific health conditions
• neutering or sending genetic material to the animal
• pregnancy – childbirth
• logbook of subsequent veterinary acts

You will need to complete:

• your personal Taxisnet credentials, if you own the pet
• the animal microchip code
